Most nodules are benign and may occur as a result of infection, inflammation, cellular proliferation, or tissue changes. They are commonly seen in the thyroid, lungs, skin, and breast tissue.

The formation of thyroid nodules is defined as small masses that develop due to the localized proliferation of cells in the thyroid gland. Iodine deficiency, hormonal changes, autoimmune thyroid diseases, and genetic predisposition can affect nodule development. Most thyroid nodules are benign, but clinical evaluation is required.

Lung nodule development refers to lesions detected in the lung tissue as round opacities smaller than 3 centimeters. Infections, previous tuberculosis, benign tumors, or rarely early-stage lung cancer may cause lung nodules. Diagnosis is usually made through imaging methods.

Subcutaneous nodule formation may occur due to an increase in fat tissue, cyst development, inflammatory processes, or formations originating from connective tissue. Lipomas, epidermal cysts, and rheumatologic nodules are among the most common examples. Clinical examination and imaging methods, when necessary, support the diagnosis.

Things You Should Know

Information

Nodule Definition

A nodule is an abnormal tissue mass that can form anywhere in the body, usually round or oval in shape, with well-defined borders. Nodule Detected in Our Body?

When you encounter the word “nodule” after a health screening or a routine medical examination, it generally refers to small lumps that appear later within the normal and smooth tissue of the body, whose boundaries can be distinguished by devices such as ultrasound or tomography. In clinical practice, nodules are usually evaluated as structures smaller than large masses. However, from a medical perspective, the most important detail is not how large this structure is, but rather which cells it consists of, how its internal structure is shaped, and how it interacts with the surrounding healthy tissues.

The structure of nodules can vary significantly depending on the characteristics of the organ in which they develop. To visualize, some nodules resemble small water balloons completely filled with fluid. In medical terminology, we call these cystic structures. Some nodules consist entirely of solid cells, like pieces of tissue or flesh, and these are called solid structures. There are also mixed structures that contain both fluid and solid components intertwined. This internal architectural difference provides very important clues about the nature of the lesion and its future behavior.

What Are the Differences Between Benign and Malignant Nodules?

Although the word nodule may be a source of concern for many people when first heard, fortunately, the vast majority of these detected structures are completely benign. Benign formations are structures that do not tend to spread uncontrollably, remain within their own boundaries, and usually do not threaten the patient’s life. Imaging devices show that the outer edges of these lesions are quite smooth, almost as if they are surrounded by a capsule.

General characteristics of benign nodules include:

Slow growth tendency

Regular and clear boundaries

Soft and flexible tissue structure

No spread to surrounding tissues

On the other hand, malignant nodules with cancerous potential may also appear in the body. Unlike benign ones, malignant cells do not follow the rules. They multiply rapidly and irregularly, have indistinct boundaries, and most importantly, tend to infiltrate surrounding healthy tissues and disrupt their structure. In addition, these malignant structures need to draw much more blood than normal by creating new vascular pathways to sustain their growth. The main goal of modern medicine is to determine as quickly and accurately as possible which of these two groups a detected lump belongs to, with minimal burden on the patient.

What Are the Main Causes of Nodule Formation in the Human Body?

The factors that cause cells to deviate from their normal functioning and proliferation cycle and form a lump are quite diverse. These factors vary greatly depending on the structure and function of the organ where the lesion is located. Our body is programmed to function in perfect balance, but when this balance is disrupted by certain internal or external signals, cellular clusters begin to form.

When looking at the general picture, the factors that trigger nodule formation include:

Past infections

Chronic inflammation

Hormonal imbalances

Genetic mutations

Environmental toxins

As a result of the body fighting a microbe for a long time, defense foci formed in that region may harden over time and become nodular. Similarly, long-term immune system responses and tissue irritations also create a basis for abnormal cell growth. Especially in endocrine glands that produce hormones, the system working less or more than necessary creates physical changes in tissues. Familial predispositions and genetic disruptions in the cell nucleus constitute the more complex dimension of the issue.

Why Are Nodules So Common in the Thyroid Gland?

Among the most common nodular structures in society are those that form in the thyroid gland located in the front part of the neck, just above the trachea. This condition is quite common especially in regions like the geography we live in, where there is insufficient iodine in the soil and drinking water. In fact, when ultrasound is performed even on healthy individuals without any complaints, thyroid nodules can be found in a large portion of the population.

The main fuel of the thyroid gland is iodine. When the body cannot get enough iodine from outside, the thyroid gland cannot produce sufficient hormones. In response, the pituitary gland in the brain constantly stimulates the thyroid to work harder. Thyroid cells exposed to continuous stimulation begin to proliferate more than normal to meet this demand, and these regional proliferations appear as swellings we call goiter and nodules within them.

Sometimes, an error occurs in the hormone production mechanisms within the thyroid gland itself. As a result of this error, some cell groups declare independence and turn into autonomously functioning “hot nodules” that continuously produce hormones independently of the body’s needs. Additionally, in chronic inflammatory conditions such as Hashimoto’s disease, where the immune system mistakenly attacks the thyroid gland, the structure of the gland is disrupted and pseudo-nodule appearances occur on ultrasound.

What Does a Nodule in the Lung Mean?

Circular spots located within the lung tissue and smaller than three centimeters in size are medically referred to as lung nodules. With the widespread adoption of check-up culture and the increasing sensitivity of tomography devices today, these small spots are detected much more frequently. This condition is particularly important in individuals who have smoked for many years, live in industrial cities with high air pollution, or are occupationally exposed to various chemical dusts.

The lungs are massive filters that are constantly in contact with the outside world. When a particle of dust, a fungal spore, or a bacterium such as tuberculosis reaches the lungs, the body’s defense cells immediately surround and trap this foreign substance. These trapped small battlefields may calcify over the years and remain in the lungs as completely harmless benign nodules called granulomas.

However, not every lung spot is so innocent. In some cases, these small structures may be the first silent sign of a cancer originating from the lung’s own cells or the spread of a malignant tumor from another part of the body to the lung. At this point, doctors make vital distinctions by evaluating how quickly the spot grows, whether its borders are smooth or spiculated, and whether it exhibits a special appearance such as ground-glass opacity.

What Factors Trigger Nodule Formation in the Liver?

The liver is the largest internal organ in our body and also the largest chemical purification facility. Nodular structures encountered in the liver may sometimes be harmless vascular tangles present from birth, while at other times they may appear as tumors formed due to the deterioration of the liver’s own cells. The rich blood supply of the liver also allows cancer cells from other parts of the body to easily reach this organ and metastasize.

Dangerous formations originating from the liver’s own tissue are usually based on chronic processes that exhaust and damage the organ over a long period of time. Viral infections such as Hepatitis B and Hepatitis C, which progress silently for many years, create continuous inflammation and damage in liver cells. While the body tries to repair this damage, scar tissue forms, and over time this condition evolves into cirrhosis. On a cirrhotic ground, the risk of errors and cancer development in constantly dividing and renewing cells is very high. Similarly, excessive alcohol consumption or severe metabolic fat accumulation are among the most important factors triggering this process.

How Can It Be Determined Whether a Nodule Is Dangerous?

When a lump is detected anywhere in the body, the first step is to use medical imaging technologies to understand the nature of this structure. Ultrasonography devices reflect the internal structure of the nodule on the screen with the help of sound waves. Sound waves produce different echoes when passing through a fluid-filled cyst compared to passing through solid tissue. If the structure appears darker compared to the surrounding healthy tissue, contains very small calcification points, or has indistinct borders as if erased with an eraser, this raises certain suspicions for physicians.

In addition, the vascular characteristics of the lesion are examined with more advanced imaging methods such as Computed Tomography and Magnetic Resonance (MRI). Rapidly proliferating malignant cells need a large amount of blood to sustain themselves. In contrast-enhanced imaging, the injected dye quickly floods into a dangerous tumor and leaves it just as quickly. This special pattern of contrast uptake, known as “washout” in medicine, is extremely guiding in the diagnostic stage. However, no matter how powerful imaging methods are, the final truth is always told by the cell itself.

Why Is Biopsy Necessary in Nodule Diagnosis?

Although modern devices provide excellent details about the shape, size, and vascularization of the lesion, there is only one way to definitively prove whether a cellular formation is completely benign or malignant. This is by taking a small sample from that formation and examining it under a microscope through the eyes of pathology specialists.

In the past, taking a tissue sample from inside the body usually required surgically opening that area. However, thanks to tremendous advances in medical science, this necessity has now been eliminated through procedures performed under the guidance of high-resolution imaging devices. Using millimetric needles, the exact center of the target lesion can be safely reached, and the most accurate diagnosis can be made without any surgical incision and without compromising the integrity of the body.

How Is Nodule Biopsy Performed with Interventional Radiology?

Although the word biopsy is often associated with a painful and difficult process among the public, today these procedures are quite comfortable and are performed under local anesthesia where only the entry point of the needle is numbed. Different needle techniques are preferred depending on the structure of the targeted organ and lesion.

The main methods applied in medical practice are:

Fine needle aspiration biopsy

Core needle biopsy

Vacuum-assisted biopsy

In sensitive organs such as the thyroid, needles even thinner than blood-drawing needles are usually used. The tip of the needle is monitored in real-time on the ultrasound screen, and it is inserted directly into the lesion, and a small amount of cells is drawn with the vacuum power of the syringe. This procedure is called fine needle aspiration biopsy and is completed within minutes without stitches.

In tissues such as the liver, lung, or breast, it is necessary to obtain samples without disrupting the arrangement and architecture of the cells within the tissue. For this purpose, needles with a special spring mechanism are used to extract a thin cylindrical tissue sample from the lesion. This method, called core needle biopsy, allows genetic tests required to determine the subtype of the tumor and to select targeted smart drugs. Vacuum-assisted biopsy, frequently used in breast lesions, is a highly advanced method that allows multiple tissue samples to be taken sequentially using vacuum power through a single needle entry.

Is It Possible to Treat Nodules Without Surgery?

Today, with the perfect integration of medicine and technology, receiving general anesthesia, making large incisions in the body, or sacrificing part or all of an organ is no longer the only option for treating some cellular enlargements. The discipline of interventional radiology has introduced very special non-surgical treatment techniques called percutaneous ablation to the medical world.

The main philosophy of these minimally invasive treatments is quite innovative. Instead of cutting out the diseased tissue with surgical instruments, a special needle is inserted into the very center of the lesion through a small hole of only a few millimeters opened in the skin under ultrasound or tomography guidance. Then, very high heat or extremely low freezing energy is delivered through the tip of this needle, destroying the diseased cells in place without removing them from the body. These cells, having lost their vitality, are gradually absorbed over months by the body’s natural immune and cleaning system, shrinking and eventually turning into a harmless scar.

How Is a Nodule Destroyed with Radiofrequency Ablation?

Radiofrequency ablation is one of the most proven and widely used thermal treatment methods in the medical world. During this procedure, high-frequency alternating electrical current is delivered through the tip of a special needle placed inside the diseased tissue. This current causes the ions inside the cells to vibrate at an incredible speed.

Just as rubbing your hands together quickly produces heat, this cellular friction creates a controlled temperature of 60 to 100 degrees Celsius at the tip of the needle. When the temperature reaches a certain level, the proteins inside the cells undergo irreversible structural changes, similar to how egg white turns from transparent to white when boiled, and the cell loses its vitality. This method is particularly successful in thyroid nodules that grow and cause shortness of breath, as well as in various liver tumors.

In Which Types of Nodules Is Microwave Ablation Preferred?

Microwave ablation technology is based on a set of physical principles similar to those used in microwave ovens at home. High-frequency electromagnetic waves sent into the tissue through the tip of the needle target the water molecules inside the cells. Under the influence of the electromagnetic field, water molecules vibrate billions of times per second, producing a very strong kinetic heat energy.

The most prominent advantage of this method is its ability to reach much higher temperatures in a much shorter time compared to radiofrequency current. In addition, if a large blood vessel passes right next to the lesion to be treated, the disadvantage of flowing blood carrying away thermal energy is less pronounced in microwave technology. Due to this powerful heating capacity, it is often considered the first choice especially in large liver tumors and lesions within the lung parenchyma.

How Is a Nodule Treated by Freezing with Cryoablation?

Cryoablation, which works on the exact opposite principle of destroying diseased cells by burning them, is based on freezing tissues using extreme cold. High-pressure argon and helium gases are passed through specially designed needles. Due to the rapid expansion of these gases, the temperature at the tip of the needle drops to freezing levels of minus 140 to minus 170 degrees Celsius within minutes.

This sudden and deep freezing process creates sharp ice crystals inside and around cancer cells. These ice crystals physically tear cell membranes and stop blood flow by rupturing the vessels feeding the tumor. The biggest advantage of the procedure is that this ice ball can be seen very clearly as a dark area on ultrasound or MRI screens. Thus, the physician can track exactly where the freezing process begins and ends. In addition, due to the natural numbing effect of extreme cold on nerve endings, pain during the procedure is almost negligible.

What Are the Advantages of Interventional Radiology Compared to Nodule Surgery?

Choosing non-surgical treatment methods instead of open surgery provides numerous benefits that greatly increase the psychological and physical comfort of patients. Patients are largely protected from the stress and potential complications of major surgical procedures.

The most prominent advantages of these modern methods are:

Use of only local anesthesia

No incision or sutures

Preservation of healthy organ tissue

Prevention of lifelong medication use

Same-day hospital discharge

No aesthetic scarring

While traditional surgery requires large incisions in the neck or abdomen, separation of tissues, and suturing at the end of the procedure; ablation is performed through only a tiny needle hole. In thyroid surgeries, since all or part of the gland is removed, patients must take thyroid hormone pills every morning for the rest of their lives. However, in needle ablation, only the diseased nodule is targeted, the healthy parts of the thyroid gland are left untouched, and the organ continues to produce its own hormone. Instead of undergoing months of recovery, patients can return to their normal lives the day after the procedure.

How Are Surrounding Tissues Protected During the Procedure?

One of the most common concerns of patients during heat-based procedures is whether this heat will damage nearby important nerves, major vessels, or the trachea. Interventional radiologists use highly advanced techniques to protect these critical structures from thermal damage.

One of these techniques is hydrodissection, which involves creating a fluid cushion. If the mass to be treated is very close to a nerve controlling the vocal cords, a safe medical fluid is injected between the mass and the nerve using a very fine needle just before the procedure. This fluid pushes the mass away from the nerve and acts as a perfect physical barrier, preventing heat energy from reaching the nerve. In addition, the procedure is not performed blindly; the whitening area where heat spreads is monitored in real time on ultrasound. In large masses, to prevent heat from accumulating in one point and damaging surrounding tissues, the needle is continuously moved within the nodule at a certain rhythm to ensure safe and even distribution of energy.

Who Are Suitable Candidates for Non-Surgical Nodule Treatment?

Although these innovative techniques can be life-saving for many patients, they are not a magic wand for every medical condition. Which patient is suitable for which treatment depends entirely on the patient’s general health condition and the cellular characteristics of the target lesion.

For example, large thyroid nodules that have been confirmed to be benign by biopsy but continue to grow, making breathing or swallowing difficult, or causing serious aesthetic concerns when viewed from the outside, are excellent candidates for this treatment. In liver and lung diseases, patients who are at risk of undergoing general anesthesia due to advanced age or accompanying heart conditions benefit from this method. Similarly, in metastatic formations that are few in number but not technically removable by surgery, non-surgical freezing or burning procedures can be safely applied.

What Is the Recovery and Follow-Up Process After Nodule Treatment?

After a successful ablation procedure, patients are usually taken to observation rooms for a few hours and kept under close monitoring. Since there is no surgical trauma, severe and unbearable pain is not expected. Only mild tenderness at the needle entry site or slight aches around the neck, which can be easily controlled with simple painkillers, may be felt. After ensuring that everything is fine during the observation period, patients are discharged walking.

The actual healing process begins after discharge through the body’s own internal dynamics. Although all the cells inside the nodule are destroyed during the procedure, it is not expected that this dead tissue will disappear instantly. The body’s cleaning cells attack this area and gradually break down and absorb the dead tissue. Therefore, the actual results of the treatment emerge over months. Ultrasound and MRI controls performed at the third, sixth, and twelfth months clearly show that the volume of the mass gradually decreases, all vitality inside it disappears, and the patient’s complaints are resolved.

Frequently Asked Questions

What is a nodule and how does it differ from a cyst or tumor?

A nodule is a small, abnormal growth or lump that can develop in various tissues or organs.
