class="ez-toc-section-end">

The trillions of cells in our body actually work in harmony like a massive orchestra. Each instrument knows when to play and when to remain silent. However, sometimes damage occurring in our genetic code — which we call mutation — disrupts this harmony. Radiation, chemical substances, cigarette smoke, viruses, or sometimes simply bad luck can cause breaks in the DNA chain. Normally, the body’s “repair” enzymes fix this damage or drive the damaged cell to self-destruction. But in the process of malignant tumor formation, the damaged cell escapes this control.

Just as a car with failed brakes accelerates downhill, these cells begin to divide uncontrollably. Initially microscopic, this disruption accumulates over time and turns into a mass. The most frightening biological characteristic of malignant tumors is that they do not “respect neighbors.” They violate the boundaries of healthy tissues, invade them, and exploit their nutrient sources. This biological chaos, if left untreated, creates a serious condition that can lead to organ failure.

What Are the Main Differences Between Malignant and Benign Tumors?

One of the most common questions we hear from our patients is: “Doctor, they said there is a mass, but is it good or bad?” This distinction is vital. Benign tumors, meaning non-cancerous tumors, are generally shy. They grow within their own capsule, push surrounding tissues aside but do not invade them. They do not have a tendency to spread to other organs. When surgically removed, they usually do not cause further problems.

Malignant tumors, on the other hand, pursue unlimited freedom. They spread into surrounding tissues like an octopus with microscopic arms. We call this “invasion.” More importantly, some rogue cells that break away from the main mass can migrate to completely different parts of the body by using blood or lymph vessels like highways. When a cell originating from breast tissue travels and settles in the lung or brain, this is called metastasis. The ability to metastasize is the most definitive evidence that a tumor is malignant. Therefore, malignancy is a disease that concerns not only its location but the entire system.

What Are the Medically Defined Types of Malignant Tumors?

When classifying malignant tumors, we look at which tissue the tumor originates from. This classification is not just an academic naming system; it is the fundamental map that determines which weapon (surgery, chemotherapy, or ablation) we will use. Each tissue’s cancer behaves differently, grows at different speeds, and spreads to different areas.

The main commonly seen groups are:

Carcinomas

Sarcomas

Leukemias

Lymphomas

Myelomas

Blastomas

To elaborate on this list; Carcinomas are the most frequently encountered group. They originate from cells (epithelium) that line the skin or the inner surfaces of our organs. Most commonly known cancers such as lung, breast, prostate, and colon cancers belong to this group. Sarcomas, on the other hand, are rarer but more stubborn; they arise from “connective tissue” elements such as bone, cartilage, fat, or muscle.

Leukemia is a disruption in the blood production process in the bone marrow; here, a solid mass does not form, but the blood itself becomes diseased. Lymphomas originate in the lymph nodes, which are the outposts of our immune system. Blastomas are generally seen in childhood and originate from immature embryonic tissues. Sometimes a benign adenoma may change its character over time and transform into a malignant adenocarcinoma; therefore, monitoring so-called “premalignant” conditions is very important.

What Are the Symptoms of Malignant Tumors Indicating That the Body Is Giving an Alarm?

Our body is actually a very intelligent machine and sends us signals when things are not going well. However, these signals can sometimes be very subtle or sometimes very loud. The symptoms of malignant tumors vary depending on where the tumor is located, how large it is, and which organs it compresses.

In general, systemic symptoms affecting the whole body include:

Unintentional rapid weight loss

Severe weakness and fatigue

Night sweats

Unexplained fever spikes

Loss of appetite

In addition to these general symptoms, complaints specific to the organ where the tumor is located are more guiding. For example, a problem in the lung may manifest as a persistent cough, while a problem in the intestine may present as a change in bowel habits.

Local and organ-specific symptoms include:

Palpable hard lumps

Coughing up blood

Blood in the stool

Blood in the urine

Non-healing skin wounds

Hoarseness

Difficulty swallowing

Newly appearing moles on the body

When you notice one of these symptoms, there is no need to immediately assume the worst-case scenario and panic; however, ignoring these signals is also not correct. Early diagnosis is the strongest weapon we have in the fight against malignant tumors.

What Are Biopsy Methods for Definitive Diagnosis and Are They Risky?

Examination and imaging (MRI, CT) give us a “suspicion,” but to make a definitive diagnosis, the tissue must be examined under a microscope. This is where “biopsy” comes into play. In the past, even for a biopsy, patients had to go to the operating room and undergo incisions. Today, thanks to Interventional Radiology, this process has completely changed.

We now perform biopsies “visually.” We use ultrasound or CT devices like navigation systems. By tracking the tip of the needle millimeter by millimeter on the screen, we reach directly into the core of the tumor without touching vessels or nerves.

Commonly used biopsy methods include:

Fine Needle Aspiration Biopsy

Tru-Cut (Core Needle) Biopsy

Vacuum-Assisted Biopsy

Bone Biopsy

Especially Tru-Cut, or core needle biopsy, is the gold standard in diagnosing malignant tumors. In this method, we obtain a thin cylindrical tissue sample using a special gun system. This sample shows the pathologist not only the cells but also their architectural arrangement within the tissue. In this way, we clearly understand the type of disease, its aggressiveness, and its genetic characteristics.

There is a very common but incorrect belief among the public that “if a knife touches it, cancer spreads.” In modern medicine, the risk of tumor spread with closed-system needle biopsies is extremely low. On the contrary, starting treatment without performing a biopsy is like shooting in the dark; the probability of missing the target is very high. The procedure is usually performed under local anesthesia, the patient does not feel pain, and can return home immediately after the procedure.

What Is Ablation, a Non-Surgical Treatment for Malignant Tumors?

Technology has advanced so much that not every malignant tumor requires major surgeries, long incisions, and intensive care processes anymore. “Percutaneous Ablation” treatments that we apply in Interventional Radiology units aim to destroy the cancer cell in its location. These methods are life-saving for patients who are not suitable for surgery, are of advanced age, or do not want to undergo surgery.

Ablation literally means “removal.” We achieve this by using heat or cold. Just like in biopsy, we place a special energy needle into the exact center of the tumor under imaging guidance.

The ablation methods we apply include:

Radiofrequency Ablation

Microwave Ablation

Cryoablation

Electroporation

Radiofrequency (RFA) and Microwave (MWA) methods are based on heat. The energy emitted from the tip of the needle causes the water molecules in the tumor tissue to vibrate, creating frictional heat. The temperature within the tissue rises to 60–100 degrees within minutes. At this temperature, the protein structures of cancer cells are disrupted, cell membranes melt, and the tumor “cooks” and dies. The body gradually clears this dead tissue and scar tissue forms in its place. Especially in tumors smaller than 3–5 centimeters in the liver, kidney, and lung, the success of this method is comparable to surgery.

Cryoablation, on the other hand, is the art of killing by freezing. By passing high-pressure gases through the needle, we reduce the tip to subzero temperatures. An “ice ball” forms within the tumor. This ice ball freezes and bursts the cells. The most advantageous aspect of cryoablation is that we can clearly see the ice ball on ultrasound or CT; thus, we can control the boundaries very well. Additionally, since cold has a natural analgesic effect, post-procedural pain is minimal.

What Are Intra-Arterial Treatments for Malignant Tumors in the Liver?

The liver holds a special place in oncology. Both cancers originating from its own tissue (HCC) and metastases from organs such as the intestine and breast are frequently seen here. However, the liver has a remarkable anatomical feature: a dual blood supply system. While healthy liver cells receive 75% of their blood from the venous system called the “Portal Vein,” malignant tumor cells predominantly (90%) use the arterial system called the “Hepatic Artery” for nutrition.

As interventional radiologists, we turn this difference into an advantage. We enter through the groin using the angiography method and advance with thin “spaghetti-like” catheters into the liver, directly to the vessel feeding the tumor. Once we reach the target, we have two different options:

Intra-arterial treatment methods include:

Transarterial Chemoembolization

Transarterial Radioembolization

Drug-Eluting Bead Therapy

In the chemoembolization (TACE) procedure, we deliver a high concentration of chemotherapy drug through that small vessel. But we do not just administer the drug; immediately afterward, we send special particles that block the vessel. In this way, we trap the drug inside the tumor and cut off its blood supply. The tumor is both poisoned and starved. Since the drug enters the systemic circulation in minimal amounts, side effects such as hair loss are not observed.

Radioembolization (TARE or Y-90) is like an “internal atomic bomb.” We send millions of microscopic spheres into the vessel. These spheres are loaded with a radioactive isotope called “Yttrium-90.” The spheres lodge in the tumor’s capillaries and emit a very high dose of radiation only a few millimeters away. While external radiotherapy is dose-limited to protect healthy liver tissue, this method allows very destructive doses to be delivered safely directly into the tumor.

How Are Cancer Pain and Other Malignant Tumor Complications Managed?

Fighting malignancy is not only about eliminating the tumor mass. Maintaining the patient’s quality of life, relieving pain, and preserving organ functions are also our responsibilities. Sometimes, even if the tumor cannot be completely eliminated, ensuring that the patient lives the remaining life comfortably is of utmost importance.

Cancers in the pancreas, stomach, and liver regions can especially cause unbearable abdominal pain by pressing on nerve plexuses in the back. Even if patients take handfuls of medication, this pain may not subside. In such cases, we perform a Celiac Ganglion Block. By entering from the back with fine needles under imaging guidance and injecting alcohol into the nerve plexus carrying the pain, we “shut down” that pathway. The majority of patients say “life feels possible again” after the procedure, and the need for morphine decreases significantly.

Additionally, malignant tumors may sometimes block the bile ducts causing jaundice or obstruct the urinary tract leading to kidney failure. To relieve these obstructions, we place drainage catheters or stents through the skin.

Other supportive procedures applied due to malignancy include:

Biliary drainage

Nephrostomy placement

Venous port placement

Ascites drainage

Pleural fluid drainage

These procedures are “palliative,” meaning they are intended to provide relief. The goal is to remove toxic fluids from the body, support nutrition, and prepare vascular access (Port) so the patient can safely receive chemotherapy.

What Are the Advantages of Interventional Radiology in the Treatment of Malignant Tumors?

The direction modern medicine is heading is toward “minimally invasive” methods that cause the least harm to the body. Interventional radiology is at the center of this philosophy. Instead of large incisions, long recovery times, and high surgical risks, it offers targeted treatments.

The main advantages these methods provide to patients include:

No surgical incision

Only local anesthesia is sufficient

Short hospital stay

Fast recovery process

Less pain

Repeatability of the procedure

No cosmetic concerns

Low risk of side effects

These methods are sometimes the only option, especially for elderly patients or those with additional diseases such as heart or lung failure, for whom general anesthesia is very risky. The patient can return home the same day or the next day and spend time with family. Moreover, cancer is unfortunately a disease that can recur. Performing a second or third surgery in the same area is difficult and risky; however, ablation or angiographic treatments can be safely repeated multiple times.

Frequently Asked Questions

What is a malignant tumor and how does it differ from a benign tumor?

A malignant tumor is a cancerous growth of abnormal cells that can invade nearby tissues and spread to distant organs.
