id="What_Is_Good_for_Knee_Pain_and_What_Is_the_Real_Cause_of_This_Pain">What Is Good for Knee Pain and What Is the Real Cause of This Pain?

To understand knee pain, we first need to look at what is happening inside. In the vast majority of patients who come to the clinic, the picture we see is a condition called “osteoarthritis” in medical terms, commonly known as calcification among the public. However, the word calcification does not really explain the condition, because it creates a mechanical impression as if the joint is rusting. In reality, the events taking place inside the knee are much more complex. Thinning and wearing of the cartilage tissue over time is of course part of this process, but the main problem is the body’s response to this damage.

While the body tries to repair the damaged area, it initiates a sterile inflammatory process in that region. We call this “inflammation.” The chemicals released during this process constantly stimulate the nerve endings inside the knee and make them hypersensitive. In other words, the problem in your knee is not just the bones rubbing against each other; it is a biological fire that continues constantly. This fire sensitizes the nerves so much that even a simple movement that would normally not cause pain is sent to your brain as a severe pain signal.

In addition, the “runner’s knee” syndrome, which we frequently encounter especially in young people or those who actively do sports, is also an important source of pain. This ache, felt behind or around the kneecap, becomes particularly noticeable when you sit for a long time in a movie theater or office, or when going up and down stairs. If this condition is not treated, it can turn into permanent osteoarthritis in the coming years.

Even more interesting is the situation of patients who have undergone knee replacement surgery. Even if the surgery is technically very successful, a significant portion of patients may continue to experience pain. This gives us a very clear message: the source of pain is not always just a mechanical disorder; the memory of the nervous system and inflammation in the soft tissues are at least as important as the bones.

The most common causes of knee pain are:

Knee osteoarthritis

Patellofemoral pain syndrome

Meniscus tears

Rheumatoid arthritis

Bursitis

Tendinitis

Kneecap dislocation

How Does Knee Cap Pain Go Away and Why Do Classical Treatments Fall Short?

When you go to the doctor, the usual roadmap is clear. First, painkillers and creams are prescribed; weight loss is recommended and you are referred to physical therapy. These are “conservative” or protective treatments and of course must be tried in the first stage. However, for a large proportion of patients, these methods unfortunately do not offer a permanent solution.

Scientific data also confirm this observation. Even in cases where medications and physical therapy are applied together, fewer than half of patients can say “yes, my pain has really gone away” after three months. Even in treatments lasting one year, although the improvement exists statistically, the difference the patient feels in daily life often remains minimal.

So why do these treatments fall short? Because most of these methods are aimed at suppressing symptoms. Painkillers temporarily block the signal going to the brain, and gels provide superficial relief. However, none of them completely extinguish the “biological fire” inside the knee or “reset” the nerves that have become exhausted and hypersensitive after carrying pain signals for years. Moreover, the negative effects of long-term painkiller use on the stomach, kidneys, and heart are an added burden. These approaches, which cannot stop the progression of the disease, put the patient into a vicious cycle: you cannot move because of pain, and as you move less your muscles weaken and the load on your knee increases, causing even more pain.

The reasons why classical treatments are insufficient are:

Persistence of inflammation

Inability to eliminate nerve sensitivity

Medication side effects

Progression of mechanical damage

Difficulty with patient compliance

What Are the Advanced Treatment Criteria for Those Asking What Is Good for Knee Pain?

Are interventional radiology methods suitable for every patient with knee pain? Of course not. In medicine, every treatment has the right patient profile. Minimally invasive, non-surgical advanced technology treatments should be considered only after certain stages have been passed. These treatments are not designed for pain that can be relieved with a simple painkiller, but for stubborn pain that seriously reduces quality of life.

First of all, the duration of pain is very important to us. We are not talking about pain that has lasted for a week or a month. We are talking about chronic pain that has become part of the body. The severity of this pain is also decisive. If you wake up from sleep at night because of pain, have difficulty doing your job, or have to give up activities you love, then you are a candidate for these treatments.

Another important criterion is the response to previous treatments. If you have tried methods such as physical therapy, medications, and intra-articular injections for at least six months and your pain still restricts you, then it is time to change strategy. These methods offer an excellent “bridge” or “exit route,” especially for patients who are not yet ready for major surgery such as knee replacement, who do not want surgery, or who are considered risky for surgery due to various health problems (heart disease, diabetes, etc.).

The eligibility criteria for advanced treatment methods are:

Pain lasting at least 6 months

High pain score

Lack of response to medication therapy

No benefit from physical therapy

Radiographically proven osteoarthritis

Inability to undergo surgery

How Does the Nerve Ablation (RFA) Method Answer the Question of How Knee Cap Pain Goes Away?

In chronic pain, the problem is often in the “communication line.” The damaged tissues in your knee constantly send a message to the brain saying “there is a problem here.” The couriers carrying this message are very thin sensory nerves called “genicular nerves.” Radiofrequency Ablation (RFA), commonly known as nerve burning, intervenes exactly in this communication line.

We can compare this procedure to temporarily cutting a telephone line. The osteoarthritis in your knee continues to exist, but the brain no longer receives that severe pain signal. Thus, you do not feel pain. The technology we use in this procedure has undergone a major evolution in recent years. In older standard methods, the needle tip would heat up too much and affect only a very small area. However, everyone’s nerve anatomy is different, like a fingerprint; even if the nerve was just millimeters away from the needle, the procedure could fail.

The “Cooled Radiofrequency” (Cooled RFA) technology we use today is much smarter. The inside of the needle is cooled by circulating water. This may sound contradictory; saying “we burn and cool at the same time” may seem strange. But the logic is this: by cooling the needle tip, we prevent tissue charring. If there is no charring, the radiofrequency energy we deliver can spread much deeper and over a wider area of tissue. Thus, we create a treatment area approximately five times larger than with the standard method. This wide area almost eliminates the chance of missing the nerve.

So how do we know that this procedure will work? This is the best part: we can do a “test drive.” Before the actual procedure, we perform a simple test called a “Diagnostic Block.” We inject a very small amount of temporary local anesthetic into the area where the targeted nerves are located.

The stages of the diagnostic block test are:

Local anesthetic injection

15-minute waiting period

Testing painful movements

Evaluation of the pain score

If after this test the patient says, “My knee feels very comfortable right now, I went up the stairs without pain,” meaning there is more than a 50% reduction in pain, we have essentially guaranteed that the RFA procedure will be successful.

How Does Vascular Embolization (GAE) Offer a Solution to the Question of What Is Good for Knee Pain?

If nerve ablation is “cutting the telephone line,” then Genicular Artery Embolization (GAE) is “putting out the fire.” During the osteoarthritis process, while the body tries to repair the damaged area, it follows an incorrect strategy and builds abnormal, unnecessary new blood vessels in that region. These new vessels, which we call “neovascularization,” do not promote healing; instead, they continuously carry inflammatory cells to the area and further provoke pain nerves. In other words, they pour fuel on the fire.

As interventional radiologists, we enter the angiography unit and, just like performing a heart angiography, access through a tiny needle-sized entry point from the groin or wrist. With very thin, hair-like catheters, we advance to the knee region. There, we identify only the abnormal vascular network responsible for pain and inflammation.

We then block these vessels by delivering microscopic particles into them. The most common concern among my patients is this: “If we cut off the blood flow to my knee, won’t my knee rot?” Absolutely not. Because the vessels we block are not the main nourishing arteries of the knee; they are vessels that formed later and are unnecessary and harmful. While healthy cartilage and bone tissue continue to receive normal nutrition, we simply shut off the “hose that feeds the fire.”

The GAE procedure can produce miraculous results especially for patients who have swelling in the knee, feel tenderness when touched, experience intense night pain, and in whom signs of inflammation are prominent. This procedure not only relieves pain but can also slow the biological progression of the disease.

The mechanisms targeted by the GAE procedure are:

Abnormal vessel formation

Synovial inflammation

Pain signal transmission

Edema formation

What Are the Advantages of Non-Surgical Methods for Those Asking How Knee Cap Pain Goes Away?

When we compare these treatments with major surgical operations, especially knee replacement surgery, there is a huge difference in terms of patient comfort. No matter how successful prosthesis surgery is, it is a major trauma in which bone is cut, the joint is removed, and artificial components are implanted. The recovery process can take months, requires painful physical therapy sessions, and carries a risk of infection.

In contrast, RFA and GAE procedures are “minimally invasive.” That is, they are performed without disrupting body integrity, without using a scalpel, and without stitches. The procedures are usually performed under local anesthesia with mild sedation, meaning there is no need for general anesthesia or being connected to a ventilator.

Our patients usually return home walking on the same day or at the latest the next day. There is no long hospital stay. The recovery process is very fast. For example, after GAE, patients can return to their daily routines within 1–2 days. After RFA, since motor nerves (nerves responsible for movement) are not affected, there is no loss of muscle strength and the patient can stand up immediately.

In addition, these methods do not eliminate your future options. If you need surgery years after these treatments, you can still undergo it without difficulty. In fact, having these treatments before surgery can even increase the success of a potential operation, because they allow the patient to go through a pain-free period and strengthen their muscles.

The main advantages of non-surgical methods are:

Scalpel-free procedure

Stitch-free application

Fast discharge

Short recovery time

Low complication risk

No need for general anesthesia

What Is the Situation Regarding Success and Durability in Treatments for What Is Good for Knee Pain?

“So doctor, how long will this treatment last for me?” is the most frequently asked and most curious question. These treatments are not a temporary enthusiasm or a placebo effect; they are backed by strong clinical studies and evidence:

In studies conducted with Cooled RFA (C-RFA), long-lasting improvements of up to 24 months (2 years) have been observed in patients’ pain scores and knee functions. More than 80% of patients report that their pain decreased by half or more in the months following the procedure. This means “I can climb stairs again” or “I can go to the park with my grandchild.”

On the vascular embolization (GAE) side, the results are similarly encouraging. In the first year after the procedure, 78% of patients experience a significant reduction in pain scores. The greatest success of GAE is that it stops or significantly delays the progression toward knee replacement surgery. Studies show that only 5.2% of patients who underwent GAE required knee replacement surgery within the following two years. This is a tremendous rate for patients who want to gain time.

Both procedures are extremely safe. Serious complication rates are negligible. After GAE, a temporary condition called “post-embolization syndrome” may occur; this includes mild fever lasting a few days, a temporary increase in knee pain, or a feeling of fatigue. This is actually a good thing, as it indicates that the body has begun to clear inflammation in that area, and it can be controlled with simple medications.

Frequently Asked Questions

What is good for knee pain and when should professional medical evaluation be considered?

Mild knee pain may improve with rest, activity modification, ice application, and appropriate exercises.
